At First Contact Clinical, we believe that good health is about more than medical care. Many factors such as housing, employment, financial pressures and lifestyle can have a significant impact on health and wellbeing.

Our specialist teams work across South Tyneside, providing personalised support that helps people improve their health, build confidence and overcome barriers that matter to them. We work alongside local NHS services, South Tyneside Council and community organisations to ensure people receive the right support at the right time.

Whether you need help with your home, returning to work, stopping smoking or accessing practical support, we're here to help.

We work exclusively within specific commissioned pathways as outlined below. This ensures our support is targeted where it is needed most, in line with the way we are currently funded.

Healthy Homes Service

Living in a cold, damp or unsafe home can have a significant impact on physical and mental health.

Our Healthy Homes team works with residents experiencing issues such as:

  • Damp and mould
  • Cold homes
  • Fuel poverty
  • Clutter or hoarding
  • Home safety concerns
  • Social isolation

We work with individuals to identify solutions, access available support and improve their living environment, helping people stay healthier and more independent.

Sleep Safe Dream Big

Every child deserves a safe and comfortable place to sleep.

Sleep Safe Dream Big supports families experiencing bed poverty by helping children and young people (0 – 18 year old) access suitable beds and sleeping arrangements.

A good night's sleep plays an important role in children's:

  • Physical health
  • Emotional wellbeing
  • School attendance
  • Learning and development

Working alongside local partners, we help ensure children have the foundations they need to thrive.

Stop Smoking Support

Quitting smoking is one of the best things you can do for your health, and you don't have to do it alone.

Our Tobacco Addiction team provides personalised stop smoking support in homes and community settings across South Tyneside.

Our experienced advisors offer:

  • One-to-one support
  • Behaviour change coaching
  • Nicotine replacement advice
  • Vaping support where appropriate
  • Ongoing encouragement throughout the quit journey

Support is tailored to each person's needs, helping people become smoke free in a way that works for them.

Trailblazer – First Contact to Progress

Our Trailblazer (Economically Inactive) programme supports adults aged 18–64 who are out of work due to long-term physical or mental health conditions, wellbeing concerns, or other life challenges.

As part of the North East Trailblazer programme, we provide personalised, trauma-informed support to help people improve their wellbeing, build confidence, overcome barriers, and take positive steps towards employment, education, training, volunteering, or other meaningful opportunities.

How we can help

Support is tailored to each individual and may include:

  • One-to-one coaching
  • Confidence and wellbeing support
  • Help to develop life skills and daily routines
  • Access to training and volunteering opportunities
  • Preparation for employment
  • Ongoing support throughout their journey

Who can access the programme?

The programme is available to people who:

  • Are aged 18–64
  • Are economically inactive
  • Have a long-term physical or mental health condition or other barriers affecting their ability to move towards employment

Waiting Well

Our Waiting Well service supports people who are waiting for planned NHS treatment or surgery to make positive lifestyle changes that can improve their overall health, prepare them for their procedure, and support a smoother recovery.

Working with one of our experienced coaches, you'll receive personalised support based on what matters most to you.

We can provide support with:

  • Stopping smoking
  • Achieving and maintaining a healthy weight
  • Improving physical activity
  • Healthy eating and nutrition
  • Managing long-term health conditions
  • Improving emotional wellbeing
  • Accessing local health and community services

Every support plan is tailored to your individual needs, helping you make realistic, sustainable changes while you wait for treatment.

Referrals

The Waiting Well service is available to eligible patients who are awaiting planned NHS treatment or surgery. Referrals are made through participating NHS services and healthcare professionals.

WorkWell South Tyneside

WorkWell South Tyneside is a free NHS service.

A WorkWell coach can listen to your needs, make a plan and put you in touch with services to help. You don't need to manage alone.

We can help you with things like:

  • Getting fitter and healthier
  • Mental health and pain
  • Money, benefits or housing
  • Talking with your employer
  • Alcohol or drug problems
  • Counselling

Who is the service for?

Our service is for adults in South Tyneside who:

  • Are struggling to stay in work because of health problems
  • Are not working because of health problems
  • Have a fit note in the last six months
  • Live in the area and have the right to work in the UK
